The Referral Process
1. Start the Conversation
Contact Trans4Mation Independent Living to ask about current availability.
2. Tell Us About the Prospective Resident
Provide basic information so we can begin determining whether our housing may be appropriate.
3. Complete Intake & Screening
When appropriate, the prospective resident will complete an intake and housing screening.
4. Coordinate Next Steps
If the housing arrangement appears to be an appropriate fit, we'll discuss availability and coordinate potential move-in planning.
